New client forms must be completed upon your initial visit. Please arrive 15 minutes early the day of your appointment to complete your paperwork. If you would like to complete your paperwork at home and return them with you on the day of your appointment, please notify me and I will be happy to email them  to you.


Fees are determined upon initial phone contact. I accept many insurance plans and Employee Assistant Program benefits. Pathway to Purpose also has a sliding scale fee making it affordable for those who want services. Fees are rendered same day services are provided.
